Dear Parents/Guardians:
Project Alpha is a National Program of Alpha Phi Alpha Fraternity, Inc. The Epsilon Lambda Chapter is hosting this particular conference for our target audience. This conference provides an environment where teens can have straight talk about the consequences of sexual behavior (e.g., teenage pregnancy and STD’s). We will also discuss relationship issues, social media safety, peer pressure, and other topics. Teens will receive information, learn problem-solving skills, and share their own experiences with other teens of both sexes.
The conference is facilitated by the men of Alpha Phi Alpha Fraternity , Inc. and local subject matter professionals. During the conference, participants will receive information on sexually transmitted diseases, teen pregnancy, abstinence/virginity, and strategies for protection. Due to the sensitive nature of the content, all conference participants under the age of 18 need the consent of their parents or guardians in order to attend the conference. No one will be admitted to the conference without this completed consent form.
Once you complete this form, the next step is that you are required to sign the forms which give your consent for your child to participate in the activities.
